3. Are there regular employee engagement surveys with self-reported demographic data?

Self-identification Form

This is a self-identification form that is used by the Canada Research Chairs Program. The data from the self-identification form is used to measure the advancement of equity, diversity, and inclusion in research in Canada. Organizations can use the research to see the different types of identity categories that can be included in self-assessment. The self-identification form also has a privacy notice statement that clearly outlines the purpose of the data, which builds trust between the organization and the applicant.
